What Is Form 1095-C?
Form 1095-C is a tax form that Applicable Large Employers (ALEs) are required to furnish to full time employees. It includes information about:
- Whether health insurance coverage was offered
- The type of coverage offered
- The cost of the coverage offered
- Whether the coverage met minimum value and affordability standards under the Affordable Care Act
Purpose of This Notice
Under current federal guidance, employers may provide a notice of availability rather than automatically distributing Form 1095-C. This notice informs you that the form is available if you need it.

To receive a copy of your Form 1095-C, you may request it through one of the following methods:
