This notice is to inform current and former employees of Dickinson Frozen Foods that Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the applicable tax year is available upon request. Form 1095-C provides information about the health insurance coverage your employer offered to you 

What Is Form 1095-C?

Form 1095-C is a tax form that Applicable Large Employers (ALEs) are required to furnish to full time employees. It includes information about:

  • Whether health insurance coverage was offered
  • The type of coverage offered
  • The cost of the coverage offered
  • Whether the coverage met minimum value and affordability standards under the Affordable Care Act

Purpose of This Notice

Under current federal guidance, employers may provide a notice of availability rather than automatically distributing Form 1095-C. This notice informs you that the form is available if you need it.
